Our Approach

We understand the challenges one faces when choosing a rehabilitation or long-term care facility for a loved one. Lake Park belongs to a family of facilities dedicated to providing the highest level of care possible for both our short-term friends, and long-term family. We implement a unique method to providing care, one that focuses on nurturing the overall mental and physical wellness of our residents, not just treating medical issues. Our state-of-the-art facilities combine cutting edge technology with a warm, friendly atmosphere. Aside from our top notch, round-the-clock medical care, guests can look forward to community outings, fun activities, luxurious rooms, and hands-on, delicate care.

For our long-term and short-term residents, we offer an individualized plan of care and services tailored to meet specialized needs. For our short-term rehabilitation patients, we focus on strengthening our patients so that they can return home as soon as possible. Family education, training, and care conferences are also available to address longer-term goals and discharge planning. In order to ensure a smooth transition after discharge, home evaluations are completed for most rehab patients returning home within about 1-2 weeks.

For our long-term patients, we do our utmost to ensure that our patients receive the best possible care. Our experienced staff maintains constant communication with our patient's medical team as well as their family members. At Lake Park, you can rest easy knowing that your family member will receive warm, compassionate care, making his or her stay with us both comfortable and enjoyable.

Testimonials

"I am glad that I was able to attend therapy at Lake Park of Madison. This is the second time that I have had to come to get rehab. It is nice to have somewhere to come and have someone that is interested in you and shows love. Everyone in therapy has taught me different ways of doing things that will help me when I get home. They have helped me become stronger and to be able to care for myself. Their love and happiness show that they care for each person. I have always cared about others and shown love. They make it a point to care for those that can’t care for themselves. Because of the staff at Lake Park of Madison I will now be able to go home by Christmas to spend time with my family."

"My favorite part of my stay at Lake Park of Madison has been therapy. When I came here after surgery and a week in the hospital I could not lift my feet off of the bed. The surgeon had told me I would not have my feet on the floor for months. Needless to say, I was depressed. I had heard good things about the therapy program here, and after the first session I knew we had made the right choice. Therapy here is more than just improving strength and mobility; it's about meeting new people as well. The therapists are friendly and encouraging. They make therapy fun by playing games and laughing a lot. It’s a lot like playing with your grandkids and it gets much better than that! I cannot thank them enough for all the help, support and encouragement I have got at Lake Park of Madison. I am now able to walk with my walker and will be able to soon return home."

- Wanda Waring, Former Patient

"The first time I came to Lake Park of Madison was from a fall I had that caused me to break my collar bone, and the second time I came was from a fall that caused me to cut my leg which became a severe wound. Each time I came to the facility I was greeted by the therapist from the Therapy Department. On both occasions they have been a great support system. They helped me to regain my strength and mobility. I was unable to walk safely after I hurt my leg. My balance was not the best. After months of therapy I am now walking with my rollator everywhere I go. No more wheelchair for me! All the therapists are caring individuals that show a little “tough love” to get you back on your feet again. They make therapy fun and enjoyable. I would recommend Lake Park of Madison’s Therapy Department to anyone that needs therapy."
