For our long-term and short-term residents, we offer an individualized plan of care and services tailored to meet specialized needs. For our short-term rehabilitation patients, we focus on strengthening our patients so that they can return home as soon as possible. Family education, training, and care conferences are also available to address longer-term goals and discharge planning. In order to ensure a smooth transition after discharge, home evaluations are completed for most rehab patients returning home within about 1-2 weeks.

For our long-term patients, we do our utmost to ensure that our patients receive the best possible care. Our experienced staff maintains constant communication with our patient's medical team as well as their family members. "The first time I came to Lake Park of Madison was from a fall I had that caused me to break my collar bone, and the second time I came was from a fall that caused me to cut my leg which became a severe wound. Each time I came to the facility I was greeted by the therapist from the Therapy Department. On both occasions they have been a great support system. They helped me to regain my strength and mobility. I was unable to walk safely after I hurt my leg. My balance was not the best. After months of therapy I am now walking with my rollator everywhere I go. No more wheelchair for me!
